Patient underwent serial debridement and VAC dressing was used to control infection.  After 15 days of VAC he was planned for free LD flap reconstruction. At surgery posterior tibia vessel was thrombosed hence anterior tibial vessel was used for flap anastomosis. After 3 weeks skin grafting was done. Recurrent Parotid tumor Defect after excision of tumors Neck dissection Parotid gland bed  ALT flap after inset 49 year old female suffered recurrent parotid tumor and developed a satellite lesion . She  came to India in search of treatment . Here Onco surgeon excised the entire tumor and satellite nodule with clear margins and ALT free flap was used for reconstruction. Free ALT flap was anastomosed to facial artery and branch of IJV. Drains were placed in anterior neck and post triangle.
