14 year old boy who suffered foot injury by roadside accident , He had medial aspect of foot shaved off.
Patient underwent serial debridement and VAC dressing was used to control infection. 

After 15 days of VAC he was planned for free LD flap reconstruction. At surgery posterior tibia vessel was thrombosed hence anterior tibial vessel was used for flap anastomosis. After 3 weeks skin grafting was done.
