49 year old female suffered recurrent parotid tumor and developed a satellite lesion . She  came to India in search of treatment . Here Onco surgeon excised the entire tumor and satellite nodule with clear margins and ALT free flap was used for reconstruction. Free ALT flap was anastomosed to facial artery and branch of IJV. Drains were placed in anterior neck and post triangle.
