Microvascular muscle flaps are very useful for treating sole injuries.Sole represents unique reconstructive challenge , muscle flaps provide good cushion to weather day today wear and tear. Sole is a weight bearing skin of body, it has to be sensate , durable and has to be absorb shock to great extent.
 40 year old male patient with traumatic wound , which was skin grafted . It broke down gradually and needed  a durable coverage. Gracillis

Foot Avulsion and LD Flap Prince

After skin grafting After VAC dressing At three weeks   First day surgery Injury after debridement After first debridement Original wound with gangrenous first toe Orthopedic fixation Removal of VAC Sponge Removal of VAC dressing At post op day 5 at discharge At discharge sole view 14 year old boy who suffered foot injury by roadside accident , He had medial aspect of foot shaved off. Patient underwent serial debridement and VAC dressing was used to control infection.  After 15 days of VAC he was planned for free LD flap reconstruction. At surgery posterior tibia vessel was thrombosed hence anterior tibial vessel was used for flap anastomosis. After 3 weeks skin grafting was done.
